No woman welcomes the news that she needs a breast lift. There is something unsettling about the prospect of having your breasts surgically altered to remove excess skin that is a result of aging, pregnancy, breastfeeding, weight changes or bad genes. But there is also such a great relief when you have completed the healing process and your new breasts have a youthful shape and firmer feel without the excess weight of sagging skin.

“Understanding that I do breast augmentation surgery through an incision under the arm is important here. My motivation is simple: leave no scar on the breast unless it is absolutely necessary. If the patient needs a lift, then I strive to place the incisions as inconspicuously as possible. In fact, when wearing a bra, bathing suit or low cut top or dress, the scars are not visible. I am well aware of my patient’s desire to avoid putting a scar on the breast and I work to reassure them that I will only do what is absolutely necessary to help meet their expectations.”

WHAT IS A BREAST LIFT?

A breast lift is a cosmetic surgery procedure done while the patient is under general anesthesia to remove excess skin and tighten the surrounding tissue to create a breast with an improved shape and firmer feel.

There are three types of incisions typically used. When a small amount of skin needs to be removed, the incision is limited to around the areola. The second incision type goes around the areola and then vertically down from the areola to the base of the breast. The third type of incision used, and the one that is necessary when a greater amount of skin needs to be removed, is around the areola with a vertical line extending down from the areola to the base of the breast and extending in either direction horizontally, along the base of the breast.

The goal is to remove excess skin from the breast, move the nipple-areolar complex higher on the breast mound and internally support the tissues to create a very natural, firmer and more shapely breast. If breast implants are also placed at the same time then the surgery is considered to be an augmentation with a lift, or an augmentation mastopexy.

By using only the smallest incision that is necessary to achieve surgical success, he is committed to creating a result that is as natural and beautiful as possible.

For example, through augmentation alone he is able to help improve the shape and feel of the breast when a the patient can compromise with a larger implant.

“If the nipple complex is in an appropriate location on the breast mound, and my patient expresses her concern over scarring, an frank discussion ensues. If she is open to the idea of a larger implant, one that will help to fill some of the excess skin and improve the feel of the breast and if she is comfortable with having a breast that is somewhat fuller and more mature, then we take this approach and elect to do the augmentation without a lift. In contrast, when I have a patient who is expecting breasts that are smaller, firmer and more perky, then I know that with those expectations she will be ultimately be happiest with a lift. Some women must have a lift, some women definitely don’t need one. But the vast majority fall into a group where the mutual decision is made based on my recommendations and assessment and her goals and expectations.” Here Dr. Hobar’s philosophy that plastic surgery should be individualized to each person is manifest.

A GOOD BREAST LIFT CANDIDATE

Most importantly, it is essential that you be in good physical health. Mastopexy is very safe and effective when the patient is in sound physical condition. It is important to disclose all of your current and past health conditions as well as any prescribed or over the counter medications or supplements you take. After a careful review of this information, Dr. Hobar might suggest certain preoperative tests to ensure that you are a good surgical candidate.

Psychological health is also an essential component. Undergoing surgery can be a stressful experience. You might need to alter certain commitments and delegate responsibility to others during the initial days of your recovery experience. It is also critical to understand that while breast lift surgery will increase the size lift your breasts and improve their shape; it is not able to solve other underlying insecurities and issues. Most people experience a myriad of emotions in the immediate periods before and after surgery. While this can be challenging, it is also normal. Being realistic about your goals and expectations is essential to achieving a positive outcome.

WHERE IS THE BREAST LIFT PERFORMED?

Dr. Hobar performs most of his cosmetic surgery in the accredited Dallas Day Surgery Center. With the latest, cutting-edge equipment and a fully trained staff specializing in cosmetic surgery, this is an ideal place for his patients. A breast lift surgery is typically scheduled for 2 to 3 hours which enables Dr. Hobar to work in a focused, precise and unhurried manner. You are put to sleep under general anesthesia by a well trained anesthetist (CRNA) or anesthesiologist. General anesthesia ensures a very well controlled, safe surgical experience. After surgery, you are taken to a recovery room where you are closely monitored as you awaken from anesthesia. While it is not necessary if you are only having a breast lift, it is a nice option to stay the night at The Cloister, a luxurious hotel attached to the Dallas Day Surgery Center where you can recuperate in a relaxing environment with 24 hour nursing care.

WHERE IS THE PROCEDURE PERFORMED?

After you have been put to sleep, Dr. Hobar will begin the surgery. During surgery, he will carefully make the incisions necessary to remove any excess skin. He will also internally lift the breast tissue to create a more improved, firmer texture. Your incisions are then closed with suture that will be removed during a followup appointment, usually 10 to 14 days after the surgery.

BREAST LIFT RECOVERY

After surgery, you will have be wearing a supportive surgical bra. There will be a simple cotton dressing applied to your incision sites. Most people experience moderate bruising and minimal swelling from breast lift surgery. The main responsibility for you as the patient during the healing process is to carefully and consistently clean your incisions. Detailed information on this protocol is included in your aftercare instructions.

The first followup appointment is typically five days after surgery. At this time, your incisions will be checked to ensure that they are being properly cared for and healing well. Ten to 14 days after surgery, you will come back for another your suture removal. At this time, if healing is progressing well, you will be instructed on the scar treatment plan that will help ensure your scars are smooth, flat and naturally colored at the end of the healing process.

Keep in mind that the healing process is just that – a process. Be patient with yourself and this process. While it is imperative to be active, eat well and get plenty of rest, it is true that people heal at different paces. Aside from taking any prescribed medicines as needed, one of the most beneficial ways to help with healing is to be active. Moving around and keeping yourself busy with household tasks is far better than laying in bed all day. Dr. Hobar is a strong proponent of early activity and if you regularly exercise, he wants you back to the gym within a week, beginning at a light activity level and gradually resuming your previous intensity.
